Session Description:
This session will provide a review of the factors that cause patients to be readmitted to the hospital shortly after discharge and effective strategies to improve the transition process.
Session Objectives:
Session participants will learn how to:
Understand the factors contributing to readmissions and their impact on the health of seniorsReview evidence-based approaches to improve care transitions
Demonstrate effective tools that can be implemented to improve the transition process
