Program Description
Telehealth can be a true "patient centered" application if used to its full potential. Telehealth brings many opportunities for care coordination so that all aspects of care can happen in real-time collaboration. Understanding the power and value of telehealth will be discussed during this session.
Learning Objectives
Upon completion of the program, participants should be able to:
- Describe how telehealth can be used for care coordination
- Explain the value of telehealth
- Explain how care teams can work together via telehealth

Technology Requirements
Participants registering for programs should have access to the following technology for best viewing and participation:
- Personal computer with Macintosh or Windows with 512 MB of RAM
- Broadband internet connection such as an office LAN, DSL, or cable modem
