Program Description
Providers still need training on clinical documentation and coding related to ICD-10-CM. Too much reliance on coding software tools and leaning too heavily on "favorites" lists can prevent clinics from accurately reporting diagnoses to patients, payers, public health and beyond. We will review Lessons Learned from the ICD-10 implementation and will highlight areas needing improvement.
In order to more fully understand documentation rules, we will perform a high-level review of the 2018 "ICD-10 Official Guidelines for Coding & Reporting" which provides the most educational benefit related to diagnosis coding. Additionally, we will review documentation rules for the most common Z-codes used in RHCs and FQHCs.
Learning Objectives
Upon completion of the program, participants will be able to:
- Know how to locate specific documentation rules for key diagnosis areas in the physical ICD-10-CM manuals. Software simply does not contain the same details!
- Understand the changes that occurred October 2017 on how to define common words found in thousands of ICD-10-CM codes.
- Identify when the sequencing and linking of diagnostic codes can affect reimbursement and proper reporting of quality metrics.
