Program Description

This session will provide a review of the factors that cause patients to be readmitted to the hospital shortly after discharge and effective strategies to improve the transition process.

Learning Objectives

Session participants will learn how to:
- Understand the factors contributing to readmissions and their impact on the health of seniors
- Review evidence-based approaches to improve care transitions
- Demonstrate effective tools that can be implemented to improve the transition process
