Program Description

The Two Georgias Initiative is a Healthcare Georgia Foundation place-based grantmaking program designed to achieve greater health equity among rural Georgians through the elimination of health disparities. The Initiative is a multi-year investment in rural communities represented by Community Health Partnerships (CHP) implementing bold and visionary Community Health Improvement Plans (CHIP) comprised of innovative solutions to better health and healthcare among their residents.

This pre-application webinar will give an overview of The Two Georgias Initiative Phase 1: Community Health Partnership (CHP) & Community Health Improvement (CHIP) RFP, the online application process and evaluation assistance provided by the Georgia Evaluation Resource Center (ERC).
