Program Description
This session will provide a comprehensive review of the challenges with using medications in seniors. Participants will learn effective strategies to ensure seniors can actively participate in managing their medications.
Learning Objectives
Session participants will learn how to:
- Understand causes and impact of Adverse Drug Events
- Describe Medication Reconciliation, when and how to perform
- Be able to use available evidence-based resources to ensure that seniors' care reflects safe prescribing of medication
